GENERAL STATEMENT OF RESPONSIBILITIES:
The overarching goal of the Circles of Care project is to plan and design a holistic, community based, coordinated system of care approach to support mental health and wellness for children, youth and families in the Tribe’s catchment area.

The Family Wellness Coordinator is the leader of the Lassen Indian Health Center’s (LIHC) Circles of Care process and effort. The position will have overall operational and management responsibility in completing the Circles of Care process and will serve as its primary spokesperson.

This position works with and around children and youth and is therefore subject to P.L. 101-630.

DUTIES INCLUDE:
1. Conduct mental health awareness training sessions.
2. Builds trusting partnerships with community members, LIHC Staff, and collaborative agencies.
3. Establish and maintain a working relationship with community agencies in coordination with services provided.
4. Assist in the development and implementation of policies and procedures.
5. Update education through courses, workshops and professional literature as it relates to behavioral health treatment and prevention, particularly in its application to Native Americans and other special populations.
6. Research grant funding sources and write proposals as needed.
7. Conduct research and write reports/articles.
8. Oversee program budget, ensure financial accountability.
9. Recognize and solve potential problems and evaluate project/program effectiveness.
10. Network with local, regional, state, Tribal and national agencies; educational institutions; nonprofit organizations; and private sector business interests for program development.
11. Using input from community organizing activities, develop innovative methods of delivery of services based on cultural perspective.
12. Responsible for supervision of all planning/administrative staff and consultants assigned to the Project to ensure it is on schedule and within budget.
13. With support of project staff, researches and drafts the project’s Strategic Plan, Circles of Care Handbook, Community Needs Assessment, and Final Report.
14. Design and implement an outcome evaluation process which will provide feedback on the impact child wellness activities have on the well-being of children and families that are served.
15. Contributes to team effort by participating in the Susanville Indian Rancheria’ facility wide program improvements, goals, and standards.
16. Other duties as assigned.

MINIMUM QUALIFICATIONS:
1. BA/BS degree in Social Work, Child Development, Sociology, Psychology, Public Administration, or a related field, with two years’ experience working in Behavioral Health Counseling Program or Health/Planning/Administration. Equivalent combinations of education and experience may be considered.
2. Experience working in a therapeutic setting with adults, adolescents and children preferred.
3. Supervisory experience preferred.
4. Ability to work independently; must be self-motivated.
5. Federal grant writing and grant administration experience highly desirable.
6. Must have a valid driver’s license with no more than five MVR points and be insurable as determined by the Susanville Indian Rancheria insurance provider.
7. Must pass a background investigation in accordance with P.L. 101-630.

As an employee of the Tribe, he/she will be subject to the Policies and Procedures of the Susanville Indian Rancheria.

PHYSICAL SAFETY:
The work requires minor physical exertion such as short periods of standing, walking, some recurring bending, crouching, stooping, stretching, reaching, or similar activities, such as setting up displays, moving tables, etc.

WORKING ENVIRONMENT:
Occasional exposure to objectionable conditions or variations such as those found in variable weather conditions.
